"cake" is the most current code. Please use that. I suspect you just ran into the hard bandwidth/QoS implementation in the older code.

I had not got around to trying the "atm"  option in cake, which tries to compensate for cell size also. So you should enable that - and I havent added support for it to sqm either!

IMO it wouldn't hurt for 'tc atm', 'tc adsl', and everything else to default to 'overhead 44'.  (The highest overhead Seb reported; the original worst case overhead[1] plus a vlan header for IPTV).
