[Cake] checkpatch cleanup is done of the sch_cake repo

Along the way I did a bit a code cleanup, renaming a few variables, notably
the pile of "fqcd" became elided and assigned to "b", which is just as
meaningful, and shorter... and classes got renamed to bins.

is there a way to run checkpatch as a hook on github and just refuse anything
that generates an error? There is no way now, without thought, anything else
out of tree on cake can be easily merged, you need this as a base.

now I have to go off and torture small, lovable, furry animals to cope with the
stress of dealing with all that pesky detail. After that, perhaps I'll
find the energy
to tackle the API changes... while simultaneously going after more big
names for the fcc signatures, AND testing 100GigE. Successfully tested
was 10GigE with soft rate shaping. Getting past 18Gbit is problematic
at 40Gbit.

I really wouldn't mind if we had a version of the codel algorithm that actually
fit into 78 columns.
