  1/ip level qdisc with qfq/sfqred and nat support

This finally more or less duplicates the prototype from
several months back, giving (hopefully) 1/ip level
of fairness... assuming qfq won't crash.

(which right now, it isn't)

qfq+sfqred at 4Mbit has about 50ms of delay and jitter, max.
My target was 5ms.
