An implementation of CoDel AQM, from Kathleen Nichols and Van Jacobson.

Based on initial work from Dave Taht.
Refactored to help future codel inclusion as plugin for other linux
qdisc (SFQ), like done with RED plugin.
Tested up to 10Gb speeds with no particular problems.

v12: algo changes after Kathleen & Van last updates
- introduction of lastcount.
- minbytes renamed to maxpacket.
- maxpacket automatically learns biggest packet size.
- ldelay record sojourn time of last dequeued packet.
- various changes, better comments...
